The Achievement Awards is an annual project of the Quezon City Science High School Alumni Association, Incorporated (QCSHSAAI) launched in September 1999.

The project was designed to identify and acknowledge the achievement of its members in their respective fields and professions that made an impact on their school, their community and the world.

 

Eligibility Requirements

 

With a period of ten (10) years that must have lapsed between graduation from Quezon City Science High School and nomination, the candidate for this award must also be a living model in his or her chosen profession or field of expertise for at least five (5) years prior to his or her nomination. The nominee must possess good moral character, a law-abiding citizen who contributes to the betterment of society.

Past awardees may no longer be nominated for the same category of awards.

 

The candidate may be considered in any of the following categories:

 

  1. COMMUNITY SERVICE

 

For individuals who have rendered outstanding work in the service of local communities for poor or disadvantaged sectors (women, children, disabled, the elderly, youth, indigenous communities) where services of the government or private sector are inadequate.

 

  1. GOVERNMENT SERVICE

 

For individuals who have rendered outstanding work in the bureaucracy, Congress, the judiciary, the military and police, or have been responsible for innovative practices and programs in government consistent with public service values such as accountability, transparency, commitment to democracy, simple living, etc.

  1. SCIENCE AND TECHNOLOGY

 

For individuals who have excelled in either the natural or basic sciences through their research, publications, teaching, consulting services, or implementation of projects and programs.

 

  1. EDUCATION

 

For individuals who have excelled in the field of education through the teaching profession, research, or extension services, which has resulted in the development of social consciousness of people, or technical expertise among students.

 

  1. ENTREPRENEURSHIP

 

For individuals who have exhibited a high degree of success in the private sector through their entrepreneurial talent or management skills, which have ensured the establishment, growth, or expansion of business enterprises.

 

  1. PRIVATE PRACTICE OF PROFESSION

For individuals who have exhibited a high degree of success in the practice of their profession, such as those but not limited to engineering, medicine, architecture, media, law, among others (may not necessarily be in line with their educational training).

 

Selection Process

 

The Quezon City Science High School Alumni Association, Incorporated invites graduates to nominate candidates for the award. The candidate signifies his/her willingness to accept the nomination. The Screening Committee, composed of the President of the QCSHSAAI, four (4) representatives from the QCSHS Alumni Association, Inc., and one (1) representative each from the Faculty and the Homecoming Committee, reviews the nominations and selects the Recipient(s) of the Award. The Screening Committee assigns the nominees to the appropriate categories and may award one or more candidates per category. The award is not limited to specific school batches or fields of endeavor. The Award is conferred during the Homecoming celebrations.

The nominees will be evaluated according to the following criteria: ACHIEVEMENTS (60%), MORAL ATTRIBUTES/STANDING IN COMMUNITY (30%), and ACADEMIC TRAINING (10%).

 

The Screening Committee shall be the undertaking body of the project. It reserves the right to pass judgment on any issue or question that may arise in connection with these rules. It shall review the entries to ensure that all requirements pertaining to nominations are complied with. The Committee will solicit feedback from fellow alumni and references provided by the nominees. The Committee shall select the awardees and recommend the names to the QCSHSAAI Board of Directors for confirmation and approval. The Board’s decision shall be final and cannot be appealed.

 

Nomination Process

 

  1. Indicate which category the candidate should be considered.
  2. Complete, sign, and date the nomination form.
  3. Include the following materials in the nomination packet:
  • Nomination Form (duly signed by the nominee, attesting to the correctness of all stated facts and giving consent to the independent verification by the Screening Committee)
  • One-page write-up about the nominee (for publication in the souvenir program, should nominee be selected as recipient of the Award )
  • Brief description of nominee’s career, including background and scope of work or activities (research, publications, trainings, etc.)
  • List of nominee’s citations, honors, and awards received (including scholarships, grants, etc.)
  • Documentation on citations, honors, and awards received
  • Additional data showing the nominee demonstrated excellence in his/her field of practice; proved leadership in his/her sphere of activities; contributed to the welfare of the school, the community, the country, and the world
  • Two (2) passport-sized colored photos
  • Three (3) character references, with complete contact details
  • Scrapbook with pictures (optional)
